For the past 5,000 years I've been using the same Math Autobiography as the first day homework assignment. Though I've enjoyed it, I've A.D.D.-ed out of it and decided to mix it up. This is my first draft of what I'll be expecting of my students this year in math: (
the worksheet). I'm curious to see what they come up with. I think I'll tell them (and follow through) that I'm doing the assignment also. Really not a sports person at all, so that's going to be a stretch for me.
I love it! Great idea. I have been using the same questionnaire for just as long and it is time for a change. I might be using this. Thanks for sharing.
ReplyDeleteYou're welcome. I'm excited to see what I get back.
ReplyDeleteI LOVE the part where the parent has to write something they are proud of their child for. I will definitely be using that!
ReplyDeleteIt IS fun to read the parent comments. But if there is NO comment, then I hope it's just a forgetful thing other than, "oh, we have NOTHING we're proud of".
ReplyDeleteI think maybe I also want to add some more parent questions ..... maybe something like: what are some qualities of your child that surprise or delight you? What personality trait does your child have that is opposite of yours? ...
I am always looking for ways to improve the information I get from students and parents. Below are two surveys I have sent out in the past.

Jake: I like the fact that yours is computer-based. I'm going to try to do that more in CS this year and not print out so much paper.
ReplyDeleteI really like this.It is a great idea.I learnt a lot..Thank you
